by calling the office of admissions or
requesting information from the university's

A limited amount of applications are
available in the guidance office.

Complete a
transcript request form
with EVERY application.

Transcript request forms are available in
the guidance office.

SUBMIT A COMPLETED APPLICATION TO YOUR
COUNSELOR.

A completed application may include:

Check for the application fee or a fee
waiver (n/a if you paid online)

Completed transcript request form
